In order to make our services accessible to individuals without private insurance, we offer three assessment packages described in more detail below:
Baseline Assessment
Abbreviated Neuropsychological Assessment
Comprehensive Neuropsychological Assessment
Our Services:
1) Baseline Assessment
The Baseline Assessment includes OHIP-covered services and provides an assessment of dementia risk as well as providing individualized lifestyle medicine intervention recommendations to promote optimal brain health.
The multi-step assessment includes:
1) an in-person intake appointment with a detailed medical history, physical exam and fitness testing
2) an in-person cognitive testing session (two hours)
3) bloodwork
4) a feedback session (virtual or in-person) with lifestyle medicine intervention recommendations
5) a three-month follow-up appointment (virtual or in-person) to discuss progress with lifestyle intervention implementation
Cost: $200 for materials and supplies for cognitive testing

Please note, our services do not negate family physicians, as our physician has a focused practice designation. Please let our team know if cost is a barrier to your participation in our program.
2) Abbreviated Neuropsychological Assessment
The Abbreviated Neuropsychological Assessment includes OHIP-insured services and an abbreviated neuropsychological assessment to provide a detailed assessment of dementia risk through neuropsychological measurement and advanced statistical analysis. Individualized lifestyle medicine intervention recommendations to promote optimal brain health will be provided.
The multi-step assessment includes:
1) an in-person intake appointment with a detailed medical history, physical exam and fitness testing
2) abbreviated neuropsychological assessment (three hours of in-person cognitive testing)
3) bloodwork
4) a neuropsychological feedback session (virtual or in-person) with lifestyle medicine intervention recommendations
5) a three-month follow-up appointment (virtual or in-person) to discuss progress with lifestyle intervention implementation
Cost: $1000 for the uninsured (non-OHIP) service of neuropsychological assessment.

3) Comprehensive Neuropsychological Assessment
The comprehensive assessment includes OHIP-insured services and a full neuropsychological assessment to provide a highly detailed assessment of dementia risk using in-depth measurement and advanced statistical analysis. Individualized lifestyle medicine intervention recommendations to promote optimal brain health will be provided.
The multi-step assessment includes:
1) an in-person intake appointment with a detailed medical history, physical exam and fitness testing
2) neuropsychological assessment (two days of in-person cognitive testing)
3) bloodwork
4) a neuropsychological feedback session (virtual or in-person) with lifestyle medicine intervention recommendations
5) a three-month follow-up appointment (virtual or in-person) to discuss progress with lifestyle intervention implementation
Cost: $4500 for the uninsured (non-OHIP) service of neuropsychological assessment.
